Patient data was siloed across dozens of providers, making coordinated care difficult in practice. Previous integration attempts stalled due to inconsistent HL7/FHIR implementations, varying levels of technical readiness among providers, and the compliance overhead involved.

We implemented a FHIR R4 gateway with HL7v2 translation for legacy systems, layered on a zero-trust network model with end-to-end encryption and patient-controlled consent. Compliance auditing was built in from the start, though aligning all 140+ providers on a consistent onboarding process was the most time-consuming part of the engagement.

Clinicians can now pull a reasonably complete patient history in seconds rather than waiting days for faxed records. The platform passed its HIPAA audit without major findings. Daily request volume has grown steadily as more providers complete onboarding.
